Requesting a repeat prescription by post

If you wish to request a repeat prescription by post you will need to fill in the form attached to your
previous prescription and send it to us. If you wish the prescription to be returned to you then you
will also need to include a stamped addressed envelope.

If the previous prescription is not available then you can still request a repeat prescription. You will need to clearly write the details of your medication (name, strength, and quantity) and send it to us.

You will need to allow up to 5 working days, providing you have sent a stamped addressed envelope.
