Register
You may register with us if you live in one of the following areas: Battlefield, Benton, Byker, Heaton, Gosforth, Jesmond, Longbenton, Sandyford, Shieldfield, Wallsend or Walker.
To register at the practice, print and complete both the new patient registration form and GMS1 form below (at the bottom of this page).
Once completed, bring them along to the practice and hand them in at reception. We will ask for the following:
All new patients who wish to register with Falcon Medical Group will be required to provide the following for your new patient health check:
1) Completed registration forms
2) Identification (see list below)
3) Urine Sample
4) B-Side or printed list of your medication
5) Copy of your child’s immunisation record
6) Warfarin book (if you take warfarin)
7) Blood monitoring book (if your bloods are monitored by a specialist / consultant.
Failure to provide any of the above may delay your registration to the practice.
Identification needed is 1 of each of the following categories:
1 Proof of identity (one of the following)
Valid Passport
Driving Licence
Birth Certificate
Identity card
1 Proof of address (one of the following)
Recent utility bill
Recent mobile phone bill
Recent Bank Statement
Recent Rent book Please ask reception for further info.
A new patient check appointment will be made for you and we'll take care of the rest such as obtaining your notes from another GP practice.
